We invite you to be a part of the Long Beach LGBTQIA2S+ Community Heatlh Needs Assessment Community Survey! A completed survey = $30 grocery gift card.

Criteria:

• 18+ years and up
• Resident of Long Beach
• Identify with the LGBTQIA2S Community

Participation is voluntary. If you choose to participate in the survey, it will take approximately 25-30 minutes of your time. You will be asked to share information about your health, opinions, and behaviors. The information shared will be used to support planning for services, resources, and additional data collection with the Long Beach LGBTQIA2S+ community. You may skip any questions you would prefer not to answer, and you may stop completing a response at any time by exiting the survey. All of your responses will be collected anonymously and will not be linked with your personal information. You will be compensated with a grocery gift card for the successful completion of a survey.

Tuesdays

• May 27: 10 – 12 PM
• June 3: 10 – 12 PM
• June 10: 10 – 12 PM

Wednesdays

• May 28 5:30 – 7:30 PM
• June 4: 5:30 – 7:30 PM
• June 11: 5:30 – 7:30 PM
